The St. Vincent's Health Home Care Coordination Program offers eligible clients with complex behavioral and medical conditions who are insured by Medicaid a variety of services, including:
Assistance and coordination with medical and psychiatric appointments and providers
- Help with financial entitlements
- Support for vocational, educational and socialization goals
- Assistance with housing
Care coordination staff can provide linkages to services and agencies throughout Westchester County to assist clients in avoiding re-hospitalizations and unnecessary use of emergency services.
Clients who do not have a behavioral health diagnosis, but who have certain medical conditions may be eligible for health home services. Those who do not have Medicaid are eligible for care coordination services if they have a diagnosis of a serious mental illness. Please call (914) 925-5060 for more information about eligibility and referrals to the program.
